Good afternoon.

We are preparing to modify our District forms.  I was wondering if you could share the following items if applicable in your District:


*         the asset addition/disposal form utilized when purchases are made or when assets are eliminated;

*         an equipment usage agreement to be signed by each employee agreeing that any equipment will not be improperly utilized;

*         and an agreement of responsibility for the safe keeping of District assets and the repercussions if they are not properly returned and accounted for or damaged during the employee's possession.
