[Mail_treas] Section 125 Plan

Good morning.

If you are a district that handles your section 125 plan in house, I was wondering if you would be willing to share the form you have employees complete/sign?  I am looking at all possibilities for next year, and a couple of districts on a previous email indicated they have their employees sign in house.
