[Mail_treas] MR-12, MR-31and AOS Fraud reporting form

We provide the CN-6 and CN-7 reports to our auditor and it seems to be what they need. These reports are either tracked by hand or generated from the lunchroom’s POS system (which is what we have – Meals Plus).

Diane,
     Julie is correct regarding the MR12 and MR31 that it is information you can get from your lunchroom supervisor. The MR12 & MR31  is no longer available in that form. The  old MR31  is the monthly transaction and summary sheet for regular lunches by building/district and the MR12 is the same information but for breakfast transactions. Good luck.

Attached is the AOS fraud reporting form.  We use Public School Works and have all employees sign off on the form annually.

The MR reports sound like food service reports but I'm not sure.
