[Mail_treas] Sick leave bank donation form request

Hi Everyone,

Our District is setting up a sick leave bank for an employee who has a
catastrophic health event. This is the first time such a bank has been
established for the first time in many, many years, or maybe for the first
time even.

Does anyone have a donation form which they would be willing to share?
