[Mail_treas] exit interview/survey

Good afternoon!

We are taking a good, long look at our employee retention efforts and abilities and would like to create an exit interview or survey for those teachers and staff members who choose to leave our district for reasons other than retirement. If you have a survey that you use and wouldn't mind sharing it would be very much appreciated.
