[Mail_treas] Supt. Licensure Verification Form

Good afternoon.

I was wondering if anyone has put together a Supt. verification form
(licensure, etc...) to go with new hires that was mentioned at the OASBO
Hot Topic seminar yesterday.  If you have one, would you be willing to
share?
