Hello Colleagues,

My dilemma...

An employee passed away and has a designated beneficiary for life insurance
and SERS.

Our OAPSE contract language states: "A member of the bargaining unit, who
is vested, and dies while employed by the Board, shall have severance paid
to his/her designated beneficiary.  The Board shall provide a designation
of beneficiary form for this purpose."

1. How do I pay the beneficiary? Budgetary?

2. Does anyone have a form for this purpose that would be willing to share?

I also plan to reach out to our auditors to see if they have an opinion as
advised by legal.
