I know of one such claim in our district for sure.  However, I am beginning
to suspect many more.  On the one employee I was certain of fraud, I faxed
an appeal to ODJFS.  It seemed like a useless appeal as I heard nothing
from them.  Then I read in one of those daily OASBO message board threads
about going to the ODJFS and filling out an online fraud report which I
did, but this was only after learning that the employee name appeared on
the Pre-Notice of Billing from ODJFS (Notice of Benefits Paid and Charges
to Employer”.  Within a few days I received the email below from ODJFS that
basically says they were putting a Stop Payment Order on said employee
benefits.  I hope you find this useful and I highly recommend utilizing
this online reporting of fraud tool that ODJFS has on their web site.

The fraudulent claim has been located per your email thread below; as a
result, a "STOP PAYMENT" has been set - NO FURTHER PAYMENTS WILL BE ISSUED.

Due to the excessive amount of fraud that is currently being reported to
our agency - our phone lines are flooded.  We are currently working
remotely and are unable to personally contact each individual claimant and
or employer via phone at this time.   There is no need to further respond.

  Explanation: We received a Request To Employer For Separation
Information.  Even after reporting said request that the Employee never
lost Employment with us, ODJFS went ahead and approved the Claim.  Employee
indicates they never filed such a claim.  We faxed an appeal of the Claim
Determination of ODJFS on 12/01/2020.  Today we received the "Notice of
Benefits Paid and Charged to Employer" - Said employee (Name removed for
security purposes) was listed on two lines of the report.

Just a heads-up.  We just received 2 claims for state unemployment that are
fraudulent.  This is the second or third time this has happened in our
district.  Part of the residual effects of COVID-19! And, no I am not a
doctor.
