We always require a W-9.

FYI - Regarding athletic departments, some of the "clubs", "boosters",
"parent groups", "friends of____ groups", etc. that athletic departments
want to pay for tournaments and such try to say they are not required to do
W-9s.  I always tell them, no W-9, no check.  I also let them know that I
would be glad to make the check out to the school district they are
representing and it can go to their athletic department as opposed to their
"club", "group", etc.
